Community Network Alliance Group, LLC (CNAG) is a multifaceted social service corporation that offer services that is supported by evidence-based practice to improve the wellbeing of our clientele and their families. Our unique and dynamic company provide solutions to the challenges faced within our healthcare and foster care systems. Our company was established July 2010 under Texas Medicaid and Health and Human Services (HHS) to underserved communities. We have expanded our services to Region 1 of Texas, and pursue growth throughout the entire state.

We strive to be a catalyst for sustainable economics in the rural community and surrounding areas. CNAG will provide resources for the underserved population. These resources include: comprehensive assessments for transitional living, individualized care plans, creative activities, residential housing, and extended case management services (ECM) to benefit and enhance life skills of young adults that have aged out of foster care. Also, CNAG will provide additional services such as hospital sitting to CPS and APS clients that have been injured, harmed, and hospitalized.

CNAG has the instinct to serve others and make positive contributions at the local and state levels by providing comprehensive services grounded by the passion to help people navigate through setbacks and overcome life’s obstacles.  The progress of each stakeholder starts the day of arrival for each new client to CNAG. History is made with the success of each client, case manager, and community partners to improve performance measures and increase the success of our community one individual at a time.

We provide sharp, motivated health professionals with the education, certifications, and professional experience to provide evidence-based practice care, with care coordination and case management services. Our real-time approach is to proactively address the immediate and deferred needs identified with individuals and their families within our underserved communities.
